Visiting your family doctor on a regular basis is key to a healthy life. Whether you're feeling the effects of the common cold or just coming in for a checkup, we want to make sure you have all the facts you need to make an educated decision.

Services we offer include:

Individual Plans

  • Kids (18 and under) – $420 a year* | $35/month** 
  • Adults (19 – 38) – $900 a year | $75/month** 
  • Adults (39 – 68) – $1,080 a year | $90 month** 
  • Seniors (69 – 95) -$1,200 a year | $100 month** 
  • Advanced Srs.(96+) – $420 a year* | $35 month** 
  • (*with accompanying adult family caregiver membership)
  • (** + processing fee)

Family Plans

  • $2500/YEAR | $210/month**
  • Up to 2 adults 
  • & 2 dependent children (age 18 & under) 
  • Additional kids 
  • $20/mo each

Office Visits

Clinic visits, during regular hours and communications (email via portal and phone) are provided without any additional fees. House calls and office visits outside regular hours can be arranged on an individual basis. We do charge a convenience fee for these additional services. Currenty, in-patient hospital services are not included in the membership packages.

Emergency Treatment

If you have a life-threatening or severe injury, call 911 or go directly to the nearest hospital emergency room.

Procedures

Procedures performed in-office are covered by the annual membership fee + wholesale cost of supplies used (e.g., sutures). Pathology services for skin biopsies, pap smears, etc. are either billed to the appropriate insurance by the rendering pathologist / laborotory or provided on a discounted fee basis. 

X-Ray and Laboratory Services

If x-ray studies are required, facilities can be utilized that can bill your insurance directly or provide services for a discounted cash price (often less expensively than available using insurance). Laboratory services will routinely be performed by Quest Diagnostics. If desired, direct billing to your insurance company can be done by the lab or a discounted cash price outside of insurance can be arranged. If a different laboratory company is required by your insurance company in order to be covered, this can be accommodated.

Health Forms and other Required Documentation

Most health forms (e.g., UIL physical exam forms, camp physicals, proof of annual physical exam, etc.) will be completed as a courtesy service for our members. Complex, detailed reports and supporting documentation will be provided for an additional fee based on complexity and time. Inquire regarding pricing.

A Note About Insurance

Health insurance has historically been to protect individuals against catastrophic financial loss associated with accidents and serious illness. In recent years, healthcare insurance has changed to include routine, preventive care. We are an independent, alternative way to receive primary health care, not a substitute for health insurance. We encourage, but do not require, our members to carry some type of health insurance in case of unexpected events that require unplanned and expensive interventions, such as surgery or hospitalization. Having a healthcare insurance policy that pays for routine, planned care and minor injury and illnesses can be expensive and is one of the many reasons healthcare insurance has become unaffordable for the average person, as well as a huge hassle in accessing that care.  

We welcome patients with any and all insurance coverage. Since we have no insurance affiliations, we are not restricted in what we offer. However, to help minimize costs to our patients, we will assist you in accessing the most affordable care based on what may or may not be covered by your insurance, if that information is available.
